Is it possible to plant a listening device in someone's pocket, and listen in to the conversation some miles away?

Should qualify the question by adding it would be listend to about 12 miles away.
Theoretically yes.
Practically, no. Bouncing the audio off a satellite or emitting it on an RF frequency would enable you to listen to it from miles away, but to do such a thing would require larger equipment than something that would fit in someones pocket.
you could do it with repeaters and the like, but even so, short of the NSA's technology and budget, you probably cannot pull it off.
